Project Research Forms

Covestro PRSEF is an affiliate fair of the Intel International Science and Engineering Fair (ISEF). Covestro PRSEF complies with ISEF rules and with Federal regulations governing research to ensure the safety and well-being of humans and animals involved in student research, as well as the safety and welfare of the students themselves. Covestro PRSEF project forms (below) have been created to use the form feature in Acrobat Reader which enables you to complete appropriate fields on the computer before printing. The fields available for on-screen completion should be completed by the student.  All fields which can be completed electronically must be completed before printing the form. Do not hand write any fields which can be completed electronically. The work done on the form can be printed but not saved. 

All other fields must be completed and signed by the appropriate persons after printing the form. All fields must be completed before the form is signed by the parent and/or teacher. Pre-signed forms may not be distributed to students.

The latest version of Adobe Acrobat Reader is available as a free download from the Adobe website opens in a new window under Get Adobe Reader.

Note: The only three form formats accepted for entry into the ISEF or its Affiliated Fairs are:

  1. Original forms
  2. Locally copied original forms
  3. Forms printed from the PDF files using Adobe Acrobat Reader.

These forms are not to be electronically transferred to the Covestro PRSEF office. All forms must be printed for completion and signatures before submission.

Note: Form 1, Form 1A including a research plan, Form 1B and the Covestro PRSEF Student Registration form are required for ALL projects.   Additional forms are required if the project involves a vertebrate animal, human beings, potentially hazardous biological agents, a controlled substance (prescription drugs, consumable ethyl alcohol or tobacco), a hazardous substance or device (firearms, explosives, radiation, lasers etc.) or if the project will be completed in a regulated research institution.

Forms for Teachers

The following forms will help you with the registration and project completion process.

Quick Reference – Required Forms opens in a new window – This is a checklist of items which must be included with your students’ registration paperwork.

Teacher’s Student Inventory Form opens in a new window – This is a list of all of your students and which forms they have submitted for review. This form MUST accompany all students’ paperwork.

Most Common Paperwork Problems  opens in a new window – All teachers and/or adult sponsors MUST review this file before submitting students' registration forms to the SRC (Scientific Review Committee).

Scientific Review Committee (SRC) Form opens in a new window – The Covestro PRSEF SRC uses this form to evaluate the students’ projects.
